- copy of original Birth Certificate from the Dept. of Vital Statisics
- original copy of Baptismal Certificate from the Church that baptised your child
- current copy of Report Card
- Immunization records

Extended School/Care (after school) - until 6:00 pm - $150.00 per month for 1 child/$25.00 per day
